I understand that bodywork (Thai bodywork and massage) is a gentle hands-on energy technique that is used for stress reduction and relaxation. 

I understand that bodywork Practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional.  

I understand that bodywork does not take the place of medical care.  

It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. I understand that this is an alternative treatment and if there are any medical concerns, I need to talk to my physician.

I understand that bodywork can compliment any medical or psychological care I may be receiving.  

I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial. I acknowledge that long term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself. 

I authorize the use of oil, lotion, and ointments to my body when necessary. 

I release Body Wisdom Sanctuary for any responsibility in case of an accident, illness, or injury. 

I acknowledge that all information I provided in this form is true and accurate.

CANCELLATION POLICY: Please note that we have a firm 24 Hour Cancellation Policy for bodywork and healing services. Appointment cancellations or changes need to be made at least 24 hours prior to the scheduled appointment. Any appointment canceled with less than 24 hours notice will result in full treatment charge. 

I authorize Body Wisdom Sanctuary to charge my credit card for services canceled with less than 24 hours notice. I understand my information will be stored in my account to secure all future appointments.
